Please bring your insurance card, completed forms, and picture identification to your first appointment.
For your initial appointment, we reserve about 40 minutes for an adult and 1 hour for a child. For adults, a dental cleaning will not be completed at this visit. At this first appointment we will complete the following:
We will collect you completed medical and dental history forms, blood pressure, any medicine ramifications, and necessary X-rays. Please inform us as soon as possible if you have any current X-rays from a previous office as there may be a frequency limitation from your insurance company which would result in additional out of packet expense. We may be able to collect your previous radiographs prior to your first appointment.
We will also collect intra oral photographs and study models if deemed necessary. At your first appointment we will also conduct a soft tissue exam to determine what type of cleaning you will need.
We will then schedule you for the dental cleaning and a complimentary consultation. Our treatment coordinator will discuss examination findings in detail, including your options for treatment, the cost for treatment, timing, and financial arrangements in relation to any benefits you may have, as well as answer any questions you may have.
We ask for your cooperation in making and keeping all appointments. Our office policy requires a notice of 2 business days to change appointments in order to avoid a $75.00 broken appointment fee.
